An honest complaints route is one unhappy patients choose – because it is faster and more likely to fix their problem than a public post. Every patient gets the same two options, equally prominent: write a Google review, or send private feedback. Never a gate, never a filter – Google stays one tap away for everyone, at every step, whatever they scored. It works anyway, because by the time a patient could post, most of them no longer want to – someone has already listened, and a human answer is on its way today.
Why 1-star reviews happen at all
Most scathing reviews are not written by patients who wanted to hurt you. They’re written by patients who had a bad morning and nowhere convenient to say so. The formal complaints procedure feels far too heavy for “the phones rang out for forty minutes”, and the front desk feels too confrontational – so the frustration goes where it’s easiest: a text box on Google, open at midnight, with no one from the practice in the room. The review isn’t really the problem. It’s the symptom of a missing channel.
The honest route, step by step
- Ask everyone the same question. A “Review us” page on your own domain – linked from texts, posters and your website – asks every patient: “How many stars would you give us today?” The tap is your own private feedback; it never posts anywhere.
- Then everyone sees the same two options, equally prominent, whatever they scored: “Write a Google review” and “Send private feedback”. No routing, no filtering – a genuinely free choice.
- The private option is a short form – what happened, and how the patient would like to be contacted – which goes straight to the practice manager. Low star ratings trigger an instant alert the moment they land.
- The public route stays visible and open at every step. A patient who wants to post on Google can, whatever they tapped. The private form is an offer, never a wall.
- Someone responds fast. This is why unhappy patients keep choosing the private route. A 2-star tap at 3pm answered by a phone call at 4pm usually ends as a resolved problem – and, quite often, as a patient more loyal than before.
Why the honest version is also the only safe version
The dishonest version of this idea – showing the Google link only to happy patients, or making the public option hard to find for unhappy ones – is called review gating, and it breaches Google’s review policies. Under the UK’s Digital Markets, Competition and Consumers Act – in force for reviews since April 2025, with CMA guidance in CMA208 – hiding negative reviews or selectively soliciting positive ones is a banned practice in its own right, not just a Google-policy issue. It also breaks trust in exactly the place a healthcare provider can least afford it. The same goes for incentives: never offer vouchers, draws or discounts for reviews of any rating. The honest route needs none of it. It works on a simple human truth: people who want a problem fixed will take the fastest path to someone who can fix it – you just have to build that path and answer it quickly.
What it means for your Google page
Your rating stops being a record of your worst mornings and starts reflecting your actual spread of experiences. That matters more than ever: since NHS.uk closed its public star-ratings in 2025, Google is the main public rating UK patients can see, 71% of patients check online reviews as the first step in finding a new doctor (Software Advice), and 46% of consumers trust reviews as much as recommendations from friends and family (BrightLocal 2023). And when the occasional harsh review still lands – it will – reply to it calmly and take it private; businesses that respond to reviews are seen as 1.7 times more trustworthy (Google/Ipsos).
The complaints route is the point, not the by-product
Fewer 1-star reviews is the visible benefit. The bigger one is that you finally hear the small problems – the phones, the parking, the running-late clinic – weekly and in writing, while they’re still small. That’s not reputation management. That’s just a practice listening properly, with the paperwork to prove it.
